Position Overview
Realize Medical is seeking a Senior Full-Stack Engineer to own the server and web systems behind Elucis, our FDA-cleared VR surgical planning platform. You will run our production services and design the new ones we build next. This is an ownership role: you will be accountable for architecture, security, and operations across the whole server side.
Key Responsibilities
- Own and operate our production web platform and cloud services.
- Lead security hardening and maintain compliance evidence.
- Design and build new server and web services that connect with our VR, desktop, and mobile apps.
- Integrate with hospital IT environments, including enterprise single sign-on.
- Review backend designs across the team and record technical decisions.
Qualifications
- 5+ years building and operating production services that hold sensitive user data
- Sound judgement in evaluating and choosing the right technology stack for the problem
- Experience developing and deploying on-premise server software in customer environments
- AWS operations experience (ECS, Lambda, IAM)
- Authentication and SSO integration (OIDC, SAML, Active Directory)
- Experience working under a compliance regime such as SOC 2, HIPAA, or ISO 13485
- Daily use of AI coding agents such as Claude Code
- Web front-end development experience
- Experience with 3D rendering in the browser (three.js, WebGL, or Unity WebGL) is a strong asset
- Experience with medical imaging, DICOM, or regulated medical software is a bonus
Ready to apply?
We don’t accept one-click applications for this role. Send your resume and a cover letter to jobs@realizemed.com with the subject “Application: Senior Full-Stack Engineer”. Applications without a letter will not be reviewed.
In your letter, tell us about:
- A production system you personally built or ran that held sensitive data. What was your part, as distinct from your team’s? Describe something that went wrong and what you changed afterward.
- Work in an environment you didn’t control, such as an on-premise deployment at a customer site or an enterprise SSO rollout with a customer’s IT team. What got in the way, and how did you get past it?
- A technology choice you made. Why did you make it, and what would you do differently today?
- How you use AI coding agents day to day. Give a concrete example, including where you check or override what they produce.
- Why this role. What draws you to owning the server side of a regulated medical product?
How we use your letter. Your letter sets the agenda for the technical interview. We will ask you to go deeper on each example: the architecture, the decisions you made, and what you’d change. Use whatever tools you like to write it, but the experience in it has to be your own. Write about what you did yourself, not what your team did around you.
